Quad C Physical Therapy has been locally owned and operated by licensed Physical Therapist Kearstin Cooley, DPT since 2018. With a lifelong belief in the importance of family and teamwork, Kearstin strives to integrate these values into the care she and her team provide to patients.

Derek Fontenot, DPT
Clinic Director/Physical Therapist
Derek is a native of Ville Platte, LA and an alumnus of Scared Heart High School, class of 2008. He obtained a Bachelor of Science in Kinesiology at LSU in 2012 and went on to receive his Doctorate of Physical Therapy from LSUHSC in Shreveport in 2015.
Derek and his wife Blaire have four children- Everett, Adele, Christian, and Hayden. When he is not at the clinic Derek enjoys spending time with his family, watching his kids play ball, hunting, and all things LSU! Derek provides quality care for general orthopedic/sports injuries and post-operative care.

Layni Vizena, PTA
Physical Therapist Assistant
Layni was born and raised in Eunice, LA. She graduated with a Bachelors Degree in Biology from LSU in 2017 and completed the Physical Therapist Assistant (PTA) Associate of Applied Science program at Louisiana Christian University in 2023.

Layni is married to Jon-Erick Vizena and they have three kids together: a late son, Callahan James, another little boy, Channing Luke, and currently expecting a baby girl in June 2026.

In her free time she enjoys reading, being with her family, and has recently discovered a love for thrift shopping.
